Helpmates Home Care is an authorized provider of NC Medicaid 1915(i) Home and Community Based Behavioral Health Services — helping adults with mental health conditions, IDD, and substance use challenges live fuller, more independent lives in their communities.
What Are 1915(i) Services?
1915(i) Home and Community Based Services (HCBS) are Medicaid-funded supports that help adults with mental illness, intellectual/developmental disabilities (IDD), or substance use disorders to live safely and independently in their communities — rather than in hospitals, group homes, or other institutional settings.
In North Carolina, 1915(i) services are managed by Local Management Entities/Managed Care Organizations (LME/MCOs) such as Trillium Health Resources, which serves many of the counties in our area. To access these services, individuals must be enrolled in NC Medicaid and receive an authorization through their LME/MCO.

Peer Support Services
Peer Support is delivered by a Certified Peer Support Specialist (CPSS) — someone who has their own lived experience with mental illness or substance use disorder and has been trained to support others on their recovery journey. This person-centered service is one of the most effective tools in behavioral health because it provides hope through genuine shared understanding.
Who Qualifies:
Adults with mental health diagnoses, substance use disorders, or co-occurring conditions who are enrolled in NC Medicaid and authorized through their LME/MCO.
What's Included:
Individual & Transitional Support (ITS)
Individual and Transitional Support (ITS) is a 1915(i) service designed to help adults with mental illness, intellectual/developmental disabilities (IDD), or traumatic brain injury (TBI) develop the skills necessary to live successfully and independently in the community. ITS is particularly valuable for individuals transitioning from institutional settings such as hospitals, group homes, or psychiatric facilities back to community-based living.
Who Qualifies:
Adults with qualifying mental health, IDD, or TBI diagnoses who are NC Medicaid beneficiaries and need support developing daily living skills or transitioning to community living.

Psychosocial Rehabilitation (PSR) is a structured, skills-based 1915(i) service that helps adults with serious mental illness (SMI) develop, restore, or maintain the skills needed to function in daily life, interact socially, and engage meaningfully in their communities. PSR is evidence-based and addresses the cognitive, emotional, and social impacts of serious mental illness.
Who Qualifies:
Adults with serious mental illness (SMI) such as schizophrenia, bipolar disorder, major depression, or other qualifying diagnoses who are NC Medicaid beneficiaries.

Call Trillium Health Resources (1-877-685-2415) or your local LME/MCO to request a behavioral health assessment. They manage access to 1915(i) services in our area.
Complete an Assessment
A care coordinator will complete a needs assessment to determine which 1915(i) services are appropriate for your situation and level of need.
Receive Service Authorization
Once your services are authorized, you'll receive a Person-Centered Plan (PCP) that outlines the specific services and frequency approved for you.
