Medicaid Accepted

NC Medicaid & Personal Care Services (PCS)

Helpmates Home Care accepts NC Medicaid for eligible home care services. If you are a Medicaid recipient seeking Personal Care Services, your primary care physician will need to complete and submit a PCS Request Form (DHB-3051) to NCLIFTSS for authorization.

Once authorized, our care coordinator will contact you to schedule your assessment and begin services. We handle the paperwork and coordination — you focus on your health.

Step 1: Get the Form

Download the PCS Request Form (DHB-3051) from the NC DHHS Medicaid website.

Step 2: Physician Completes Form

Have your primary care physician complete the form and fax it directly to NCLIFTSS for review.

Step 3: Contact Helpmates

Once you receive your authorization, call us. We'll handle the rest and get your care started quickly.
